Failure in the intelligence cycle or intelligence failure, is the outcome of the inadequacies within the intelligence cycle. The intelligence cycle itself consists of six steps that are constantly in motion: requirements, collection, processing and exploitation, analysis and production, dissemination and consumption, and feedback. Collection The collection of intelligence is through five methods: Human Intelligence (HUMINT), Signals Intelligence (SIGINT), Image Intelligence (IMINT), Open Source Intelligence (OSINT), and Measures and Signature Intelligence ( MASINT). It is common to rely on technology when performing collection, however it can fail and cause more problems than it solves. The intelligence cycle is an idealized model of how intelligence (information gathering), intelligence is processed in civilian and military intelligence agency, intelligence agencies, and law enforcement organizations. It is a closed path (graph theory), path consisting of repeating vertex (graph theory), nodes, which (if followed) will result in Intelligence assessment, finished intelligence. Cognitive Biases
A cognitive bias is a systematic pattern of deviation from norm or rationality in judgment. Individuals create their own "subjective reality" from their perception of the input. An individual's construction of reality, not the objective input, may dictate their behavior in the world. Thus, cognitive biases may sometimes lead to perceptual distortion, inaccurate judgment, illogical interpretation, and irrationality. While cognitive biases may initially appear to be negative, some are adaptive. They may lead to more effective actions in a given context. Self-serving Bias
A self-serving bias is any cognitive or perceptual process that is distorted by the need to maintain and enhance self-esteem, or the tendency to perceive oneself in an overly favorable manner. It is the belief that individuals tend to ascribe success to their own abilities and efforts, but ascribe failure to external factors. When individuals reject the validity of negative feedback, focus on their strengths and achievements but overlook their faults and failures, or take more credit for their group's work than they give to other members, they are protecting their self-esteem from threat and injury. These cognitive and perceptual tendencies perpetuate illusions and error, but they also serve the self's need for esteem. Conformity or conformism is the act of matching attitudes, beliefs, and behaviors to social group, group norms, politics or being like-minded. The term "need to know" (alternatively spelled need-to-know), when used by governments and other organizations (particularly those related to military or intelligence), describes the restriction of data which is considered very confidential and sensitive. Under need-to-know restrictions, even if one has all the necessary official approvals (such as a security clearance) to access certain information, one would not be given access to such information, or read into a clandestine operation, unless one has a specific ''need to know''; that is, access to the information must be necessary for one to conduct one's official duties.
