Cricohyoidopexy (CHP) is a surgical procedure performed after partial removal of the larynx due to laryngeal cancer. It is categorized under horizontal laryngeal surgeries, alongside cricohyoidoepiglottopexy (CHEP), which particularly preserves the suprahyoid epiglottis, performed due to especially for tumors in the glottic region (where the vocal cords are located). The procedures aim to achieve oncological control while preserving essential laryngeal functions such as breathing, swallowing, and phonation. History Cricohyoidopexy (CHP) was first described by E. Majer and W. Rieder in 1959 as a form of supracricoid partial laryngectomy. Larynx
The larynx (), commonly called the voice box, is an organ (anatomy), organ in the top of the neck involved in breathing, producing sound and protecting the trachea against food aspiration. The opening of larynx into pharynx known as the laryngeal inlet is about 4–5 centimeters in diameter. Aryepiglottic Fold
The aryepiglottic folds are triangular folds of mucous membrane of the larynx. They enclose ligamentous and muscular fibres. They extend from the lateral borders of the epiglottis to the arytenoid cartilages, hence the name 'aryepiglottic'. They contain the aryepiglottic muscles and form the upper borders of the quadrangular membrane. They have a role in growling as a form of phonation. They may be narrowed and cause stridor, or be shortened and cause laryngomalacia. Structure The aryepiglottic folds are triangular. Polyglycolide or poly(glycolic acid) (PGA), also spelled as polyglycolic acid, is a biodegradable, thermoplastic polymer and the simplest linear, aliphatic polyester. It can be prepared starting from glycolic acid by means of polycondensation or ring-opening polymerization. PGA has been known since 1954 as a tough fiber-forming polymer. Vicryl (polyglactin 910) is an absorbable, synthetic, usually braided surgical suture, suture, manufactured by Ethicon Inc., a subsidiary of Johnson and Johnson. Superior Laryngeal Nerve
The superior laryngeal nerve is a branch of the vagus nerve. It arises from the middle of the inferior ganglion of the vagus nerve and additionally receives a sympathetic branch from the superior cervical ganglion. Thyroarytenoid Muscle
The thyroarytenoid muscle is a broad, thin muscle that forms the body of the vocal fold and that supports the wall of the ventricle and its appendix. It functions to shorten the vocal folds. Structure It arises in front from the lower half of the angle of the thyroid cartilage, and from the middle cricothyroid ligament. Vestibular Fold
The vestibular fold (ventricular fold, superior or false vocal cord) is one of two thick folds of mucous membrane, each enclosing a narrow band of fibrous tissue, the vestibular ligament, which is attached in front to the angle of the thyroid cartilage immediately below the attachment of the epiglottis, and behind to the antero-lateral surface of the arytenoid cartilage, a short distance above the vocal process.
