Fernandez Reaction
The Fernandez reaction is a reaction that occurs to signal a positive result in the lepromin skin test for leprosy. The reaction occurs in the skin at the site of injection if the body possesses antibodies to the Dharmendra antigen, one of the antigens found in Mycobacterium leprae, the bacteria that causes leprosy. The reaction occurs via a delayed-type hypersensitivity mechanism. This reaction occurs within 48 hours of injection of lepromin and is seen in only tuberculoid forms of leprosy. This represents a delayed-type hypersensitivity reaction. In contrast, the Mitsuda reaction (delayed granulomatous lesion) occurs 3–4 weeks after injection of lepromin and is only seen in patients with the tuberculoid form of leprosy (not the lepromatous form, in which the body does not mount a strong response against the bacterium). Lepromin
The lepromin skin test is used to determine what type of leprosy a person is infected with. It involves the injection of a standardized extract of the inactivated "''leprosy bacillus''" (''Mycobacterium leprae'' or "Hansen's bacillus") under the skin. It is not recommended as a primary mode of diagnosis. Method Positive reaction means: A) or more induration after 48hrs B) or above nodule after 21 days. An extract sample of inactivated Hansen's bacillus is injected just under the skin, usually on the forearm, so that a small lump pushes the skin upward. The lump indicates that the antigen has been injected at the correct depth. The injection site is labeled and examined at 48hours and 21 days later to see if there is a reaction. People with dermatitis or other skin irritations should have the test performed on an unaffected part of the body. Leprosy
Leprosy, also known as Hansen's disease (HD), is a long-term infection by the bacteria ''Mycobacterium leprae'' or ''Mycobacterium lepromatosis''. Infection can lead to damage of the nerves, respiratory tract, skin, and eyes. An antibody (Ab), also known as an immunoglobulin (Ig), is a large, Y-shaped protein used by the immune system to identify and neutralize foreign objects such as pathogenic bacteria and viruses. The Dharmendra antigen is a test widely used in India. The antigen is a suspension of de-fatted leprosy bacilli which has been extracted with chloroform-ether and killed, either by heat, or by other effective methods. In immunology, an antigen (Ag) is a molecule or molecular structure or any foreign particulate matter or a pollen grain that can bind to a specific antibody or T-cell receptor. Hypersensitivity
Hypersensitivity (also called hypersensitivity reaction or intolerance) refers to undesirable reactions produced by the normal immune system, including allergies and autoimmunity. They are usually referred to as an over-reaction of the immune system and these reactions may be damaging and uncomfortable. Tuberculin
Tuberculin, also known as purified protein derivative, is a combination of proteins that are used in the diagnosis of tuberculosis. This use is referred to as the tuberculin skin test and is recommended only for those at high risk. Reliable administration of the skin test requires large amounts of training, supervision, and practice. Injection is done into the skin. After 48 to 72 hours, if there is more than a five to ten millimeter area of swelling, the test is considered positive. Common side effects include redness, itchiness, and pain at the site of injection. Allergic reactions may occasionally occur.
