Cosmos
The cosmos (, ; ) is an alternative name for the universe or its nature or order. Usage of the word ''cosmos'' implies viewing the universe as a complex and orderly system or entity. The cosmos is studied in cosmologya broad discipline covering scientific, religious or philosophical aspects of the cosmos and its nature. Religious and philosophical approaches may include the cosmos among spiritual entities or other matters deemed to exist outside the physical universe. Etymology The verb wikt:κοσμέω, κοσμεῖν (''kosmein'') meant generally "to dispose, prepare", but especially "to order and arrange (troops for battle), to set (an army) in array"; also "to establish (a government or regime)", "to adorn, dress" (especially of women). Thus ''kosmos'' meant "ornaments, decoration" (compare ''kosmokomes'' "dressing the hair," and cosmetic). The philosopher Pythagoras used the term ''kosmos'' for the order of the universe. Anaxagoras further introduced the concept of a C ...

Electronic Medical Record
An electronic health record (EHR) is the systematized collection of electronically stored patient and population health information in a digital format. These records can be shared across different health care settings. Records are shared through network-connected, enterprise-wide information systems or other information networks and exchanges. EHRs may include a range of data, including demographics, medical history, medication and allergies, immunization status, laboratory test results, radiology images, vital signs, personal statistics like age and weight, and billing information. For several decades, EHRs have been touted as key to increasing quality of care. EHR combines all patients' demographics into a large pool, which assists providers in the creation of "new treatments or innovation in healthcare delivery" to improve quality outcomes in healthcare. Combining multiple types of clinical data from the system's health records has helped clinicians identify and stratify ch ...

Cosmic Top Secret
Classified information is confidential material that a government deems to be sensitive information which must be protected from unauthorized disclosure that requires special handling and dissemination controls. Access is restricted by law or regulation to particular groups of individuals with the necessary security clearance with a need to know. A formal security clearance is required to view or handle classified material. The clearance process requires a satisfactory background investigation. Documents and other information must be properly marked "by the author" with one of several (hierarchical) levels of sensitivity—e.g. Confidential (C), Secret (S), and Top Secret (S). All classified documents require designation markings on the technical file which is usually located either on the cover sheet, header and footer of page. The choice of level is based on an impact assessment; governments have their own criteria, including how to determine the classification of an informat ...
