Person-centered Care
In health care, person-centered care is a practice in which patients actively participate in their own medical treatment in close cooperation with their health professionals. Sometimes relatives are also included in creating the health plan. The person-centered model of health care is used both for in and outpatient settings, emergency care, palliative care as well as in rehabilitation. Background The concept of person-centered care can be distinguished from a traditional treatment model which views the patient as a passive receiver of a medical intervention. Many health professionals are traditionally focused on the needs of the patients instead of their resources. Health Literacy
Health literacy is the ability to obtain, read, understand, and use healthcare information in order to make appropriate health decisions and follow instructions for treatment. There are multiple definitions of health literacy, in part, because health literacy involves both the context (or setting) in which health literacy demands are made (e.g., health care, media, internet or fitness facility) and the skills that people bring to that situation. Since health literacy is a primary contributing factor to health disparities, it is a continued and increasing concern for health professionals. The 2003 National Assessment of Adult Literacy (NAAL) conducted by the US Department of Education found that 36% of participants scored as either "basic" or "below basic" in terms of their health literacy and concluded that approximately 80 million Americans have limited health literacy. Patient-centered Care
Patient participation is a trend that arose in answer to medical paternalism. Informed consent is a process where patients make decisions informed by the advice of medical professionals. In recent years, the term "patient participation" has been used in many different contexts. These include, for example: shared decision making, participatory medicine, health consumerism, and patient-centered care. Professor Lord Darzi's review of the NHS in 2008 introduced the idea of personal health budgets (PHBs) in the English National Health Service. Since October 2014 people eligible for NHS Continuing Healthcare were given the legal right to have a personal health budget. Health advocacy or health activism encompasses direct service to the individual or family as well as activities that promote health and access to health care in communities and the larger public. NHS England
NHS England, officially the NHS Commissioning Board, is an executive non-departmental public body of the Department of Health and Social Care.
