Hazop
A hazard and operability study (HAZOP) is a structured and systematic examination of a complex plan or operation in order to identify and evaluate problems that may represent risks to personnel or equipment. The intention of performing a HAZOP is to review the design to pick up design and engineering issues that may otherwise not have been found. The technique is based on breaking the overall complex design of the process into a number of simpler sections called 'nodes' which are then individually reviewed. It is carried out by a suitably experienced multi-disciplinary team (HAZOP) during a series of meetings. The HAZOP technique is qualitative, and aims to stimulate the imagination of participants to identify potential hazards and operability problems. Structure and direction are given to the review process by applying standardised guide-word prompts to the review of each node. Trevor Kletz
Trevor Asher Kletz, OBE, FREng, FRSC, FIChemE (23 October 1922–31 October 2013) was a prolific British author on the topic of chemical engineering safety. He is credited with introducing the concept of inherent safety, and was a major promoter of Hazop. He is listed in The Palgrave Dictionary of Anglo-Jewish History. Early life and education Kletz was born 23 October 1922 in Darlington of Jewish parents, from a Russian immigrant background. He attended The King's School, Chester, then the University of Liverpool, where he graduated in chemistry in 1944 and joined ICI the same year. During the Second World War, he was a member of the Home Guard. On 28 October 1958 he married Denise V. A hazard analysis is used as the first step in a process used to assess risk. The result of a hazard analysis is the identification of different types of hazards. A hazard is a potential condition and exists or not (probability is 1 or 0). It may, in single existence or in combination with other hazards (sometimes called events) and conditions, become an actual Functional Failure or Accident (Mishap). The way this exactly happens in one particular sequence is called a scenario. This scenario has a probability (between 1 and 0) of occurrence. Often a system has many potential failure scenarios. It also is assigned a classification, based on the worst case severity of the end condition. Risk is the combination of probability and severity. Hazard analysis and critical control points, or HACCP (), is a systematic preventive approach to food safety from biological, chemical, and physical hazards in production processes that can cause the finished product to be unsafe and designs measures to reduce these risks to a safe level. In this manner, HACCP attempts to avoid hazards rather than attempting to inspect finished products for the effects of those hazards. A process flow diagram (PFD) is a diagram commonly used in chemical and process engineering to indicate the general flow of plant processes and equipment. The PFD displays the relationship between ''major'' equipment of a plant facility and does not show minor details such as piping details and designations.
