Engineering disasters often arise from shortcuts in the design process. Engineering is the science and technology used to meet the needs and demands of society. These demands include buildings, aircraft, vessels, and computer software. In order to meet society’s demands, the creation of newer technology and infrastructure must be met efficiently and cost-effectively. To accomplish this, managers and engineers need a mutual approach to the specified demand at hand. This can lead to shortcuts in engineering design to reduce costs of construction and fabrication. Hurricane Katrina was a destructive Category 5 Atlantic hurricane that caused over 1,800 fatalities and $125 billion in damage in late August 2005, especially in the city of New Orleans and the surrounding areas. North British Railway
The North British Railway was a British railway company, based in Edinburgh, Scotland. It was established in 1844, with the intention of linking with English railways at Berwick. The line opened in 1846, and from the outset the company followed a policy of expanding its geographical area, and competing with the Caledonian Railway in particular. In doing so it committed huge sums of money, and incurred shareholder disapproval that resulted in two chairmen leaving the company. Nonetheless the company successfully reached Carlisle, where it later made a partnership with the Midland Railway. It also linked from Edinburgh to Perth and Dundee, but for many years the journey involved a ferry crossing of the Forth and the Tay. Ethiopian Airlines Flight 302
Ethiopian Airlines Flight 302 was a scheduled international passenger flight from Addis Ababa Bole International Airport in Ethiopia to Jomo Kenyatta International Airport in Nairobi, Kenya. On 10 March 2019, the Boeing 737 MAX 8 aircraft which operated the flight crashed near the town of Bishoftu six minutes after takeoff, killing all 157 people aboard. Lion Air Flight 610 was a scheduled domestic passenger flight from Soekarno–Hatta International Airport, Jakarta to Depati Amir Airport, Pangkal Pinang in Indonesia. The MIM-104 Patriot is a surface-to-air missile (SAM) system, the primary of its kind used by the United States Army and several allied states. It is manufactured by the U.S. defense contractor Raytheon and derives its name from the radar component of the weapon system. The Therac-25 was a computer-controlled radiation therapy machine produced by Atomic Energy of Canada Limited (AECL) in 1982 after the Therac-6 and Therac-20 units (the earlier units had been produced in partnership with of France). TAURUS (Transfer and Automated Registration of Uncertified Stock) was a program that set out to transfer settlements of London Stock Exchange shares from transmission of paper share certificates to an automated system. TAURUS was intended to reduce the time taken and the cost of settlement, increasing convenience and reducing settlement risk. Ariane flight V88 was the failed maiden flight of the Arianespace Ariane 5 rocket, vehicle no. 501, on 4 June 1996. It carried the Cluster spacecraft, a constellation of four European Space Agency research satellites. The launch ended in failure due to multiple errors in the software design: dead code, intended only for Ariane 4, with inadequate protection against integer overflow led to an exception handled inappropriately, halting the whole otherwise unaffected inertial navigation system.
