Engineering Disaster
Engineering disasters often arise from shortcuts in the design process. Engineering is the science and technology used to meet the needs and demands of society. These demands include buildings, aircraft, vessels, and computer software. In order to meet society’s demands, the creation of newer technology and infrastructure must be met efficiently and cost-effectively. To accomplish this, managers and engineers need a mutual approach to the specified demand at hand. This can lead to shortcuts in engineering design to reduce costs of construction and fabrication. Hurricane Katrina
Hurricane Katrina was a destructive Category 5 Atlantic hurricane that caused over 1,800 fatalities and $125 billion in damage in late August 2005, especially in the city of New Orleans and the surrounding areas. It was at the time the costliest tropical cyclone on record and is now tied with 2017's Hurricane Harvey. The storm was the twelfth tropical cyclone, the fifth hurricane, and the third major hurricane of the 2005 Atlantic hurricane season, as well as the fourth-most intense Atlantic hurricane on record to make landfall in the contiguous United States. North British Railway
The North British Railway was a British railway company, based in Edinburgh, Scotland. It was established in 1844, with the intention of linking with English railways at Berwick. The line opened in 1846, and from the outset the company followed a policy of expanding its geographical area, and competing with the Caledonian Railway in particular. In doing so it committed huge sums of money, and incurred shareholder disapproval that resulted in two chairmen leaving the company. Nonetheless the company successfully reached Carlisle, where it later made a partnership with the Midland Railway. It also linked from Edinburgh to Perth and Dundee, but for many years the journey involved a ferry crossing of the Forth and the Tay. Ashtabula ( ) is a city in Ashtabula County, Ohio, Ashtabula County, Ohio, United States, and the center of the United States micropolitan area, Ashtabula micropolitan area. Boeing 737 MAX Groundings
The Boeing 737 MAX passenger airliner was grounded worldwide between March 2019 and December 2020longer in many jurisdictionsafter 346 people died in two crashes: Lion Air Flight 610 on October 29, 2018 and Ethiopian Airlines Flight 302 on March 10, 2019. The U.S. Federal Aviation Administration (FAA) resisted grounding the aircraft until March 13, 2019, when it received evidence of accident similarities. By then, 51 other regulators had already grounded the plane, and by March 18, 2019, all 387 aircraft in service were grounded. Ethiopian Airlines Flight 302
Ethiopian Airlines Flight 302 was a scheduled international passenger flight from Addis Ababa Bole International Airport in Ethiopia to Jomo Kenyatta International Airport in Nairobi, Kenya. On 10 March 2019, the Boeing 737 MAX 8 aircraft which operated the flight crashed near the town of Bishoftu six minutes after takeoff, killing all 157 people aboard. Flight 302 is Ethiopian Airlines's deadliest accident to date, surpassing the fatal hijacking of Flight 961 resulting in a crash near the Comoros in 1996. It is also the deadliest aircraft accident to occur in Ethiopia, surpassing the crash of an Ethiopian Air Force Antonov An-26 in 1982, which killed 73. Lion Air Flight 610
Lion Air Flight 610 was a scheduled domestic passenger flight from Soekarno–Hatta International Airport, Jakarta to Depati Amir Airport, Pangkal Pinang in Indonesia. On 29 October 2018, the Boeing 737 MAX operating the route crashed into the Java Sea 13 minutes after takeoff, killing all 189 passengers and crew. It was the first major accident involving the new Boeing 737 MAX series of aircraft, introduced in 2017, and the highest death toll of any accident or incident involving a 737 series aircraft, surpassing Air India Express Flight 812 in 2010. It was the deadliest accident in Lion Air's history, surpassing the 2004 Lion Air Flight 538 that killed 25, and the second deadliest aircraft accident in Indonesia behind Garuda Indonesia Flight 152. Patriot Missile
The MIM-104 Patriot is a surface-to-air missile (SAM) system, the primary of its kind used by the United States Army and several allied states. It is manufactured by the U.S. defense contractor Raytheon and derives its name from the radar component of the weapon system. The AN/MPQ-53 at the heart of the system is known as the "Phased Array Tracking Radar to Intercept on Target", or PATRIOT. The Patriot system replaced the Nike Hercules system as the U.S. Army's primary High to Medium Air Defense (HIMAD) system and replaced the MIM-23 Hawk system as the U.S. Army's medium tactical air defense system. Therac-25
The Therac-25 was a computer-controlled radiation therapy machine produced by Atomic Energy of Canada Limited (AECL) in 1982 after the Therac-6 and Therac-20 units (the earlier units had been produced in partnership with of France). It was involved in at least six accidents between 1985 and 1987, in which patients were given massive overdoses of radiation. Because of concurrent programming errors (also known as race conditions), it sometimes gave its patients radiation doses that were hundreds of times greater than normal, resulting in death or serious injury. Settlement is the "final step in the transfer of ownership involving the physical exchange of securities or payment". After settlement, the obligations of all the parties have been discharged and the transaction is considered complete. In the context of securities, settlement involves their delivery to the beneficiary, usually against ( in simultaneous exchange for) payment of money, to fulfill contractual obligations, such as those arising under securities trades. Nowadays, settlement typically takes place in a central securities depository. In the United States, the settlement date for marketable stocks is usually 2 business days or T+2 after the trade is executed, and for listed options and government securities it is usually 1 day after the execution. TAURUS (Transfer and Automated Registration of Uncertified Stock) was a program that set out to transfer settlements of London Stock Exchange shares from transmission of paper share certificates to an automated system. TAURUS was intended to reduce the time taken and the cost of settlement, increasing convenience and reducing settlement risk. It was started in the 1980s and was intended to replace the TALISMAN system, which was only available for settlement of trades between participating brokerage firms. TAURUS was also meant to change the London Stock Exchange settlement from a two week account period, settling all trades that occurred within each two-week interval in one batch, to a rolling settlement system, settling all trades three business days after the date of the trade. Ariane 5 Flight 501
Ariane flight V88 was the failed maiden flight of the Arianespace Ariane 5 rocket, vehicle no. 501, on 4 June 1996. It carried the Cluster spacecraft, a constellation of four European Space Agency research satellites. The launch ended in failure due to multiple errors in the software design: dead code, intended only for Ariane 4, with inadequate protection against integer overflow led to an exception handled inappropriately, halting the whole otherwise unaffected inertial navigation system. This caused the rocket to veer off its flight path 37 seconds after launch, beginning to disintegrate under high aerodynamic forces, and finally self-destructing via its automated flight termination system.
