In anatomy, the abdominal wall represents the boundaries of the abdominal cavity. The abdominal wall is split into the anterolateral and posterior walls. There is a common set of layers covering and forming all the walls: the deepest being the visceral peritoneum, which covers many of the abdominal organs (most of the large and small intestines, for example), and the parietal peritoneum- which covers the visceral peritoneum below it, the extraperitoneal fat, the transversalis fascia, the internal and external oblique and transversus abdominis aponeurosis, and a layer of fascia, which has different names according to what it covers (e.g., transversalis, psoas fascia). Urinary Bladder
The urinary bladder, or simply bladder, is a hollow organ in humans and other vertebrates that stores urine from the kidneys before disposal by urination. In humans the bladder is a distensible organ that sits on the pelvic floor. Urine enters the bladder via the ureters and exits via the urethra. In human anatomy, the median umbilical ligament is an unpaired midline ligamentous structure upon the lower inner surface of the anterior abdominal wall. It is covered by the median umbilical fold. The median umbilical ligament represents the remnant of the fetal urachus. It extends from the apex of the bladder to the umbilicus, on the deep surface of the anterior abdominal wall. The medial umbilical ligament represents one of the five ligaments of the internal anterior abdominal wall inferior to the umbilicus; laterally on either side of it are one medial umbilical ligament and finally one lateral umbilical ligament. Development The median umbilical ligament begins as the allantois in the embryonic period. It then becomes the urachus in the fetus. This later develops into the median umbilical ligament at birth. Urachus
The urachus is a fibrous remnant of the allantois, a canal that drains the urinary bladder of the fetus that joins and runs within the umbilical cord. The fibrous remnant lies in the space of Retzius, between the transverse fascia anteriorly and the peritoneum posteriorly. Development The part of the urogenital sinus related to the bladder and urethra absorbs the ends of the Wolffian ducts and the associated ends of the renal diverticula. This gives rise to the trigone of the bladder and part of the prostatic urethra. The remainder of this part of the urogenital sinus forms the body of the bladder and part of the prostatic urethra. The apex of the bladder stretches and is connected to the umbilicus as a narrow canal. This canal is initially open, but later closes as the urachus goes on to definitively form the median umbilical ligament. Medial Umbilical Fold
The medial umbilical ligament (or cord of umbilical artery, or obliterated umbilical artery) is a paired structure found in human anatomy. It is on the deep surface of the anterior abdominal wall, and is covered by the medial umbilical folds (''plicae umbilicales mediales''). It is different from the median umbilical ligament, a structure that represents the remnant of the embryonic urachus. Origins It represents the remnant of the umbilical arteries, which serves no purpose in humans after birth, except for the initial part that becomes the adult superior vesical artery. The occluded part of umbilical artery becomes the medial umbilical ligament postnatal. The medial umbilical ligament arises from the anterior division of the internal iliac artery. Umbilical Artery
The umbilical artery is a paired artery (with one for each half of the body) that is found in the abdominal and pelvic regions. In the fetus, it extends into the umbilical cord. Structure Development The umbilical arteries supply deoxygenated blood from the fetus to the placenta. Although this blood is typically referred to as deoxygenated, this blood is fetal systemic arterial blood and will have the same amount of oxygen and nutrients as blood distributed to the other fetal tissues. There are usually two umbilical arteries present together with one umbilical vein in the umbilical cord. The umbilical arteries surround the urinary bladder and then carry all the deoxygenated blood out of the fetus through the umbilical cord. Lateral Umbilical Fold
The lateral umbilical fold overlies the inferior epigastric artery (a branch of the external iliac artery) and its accompanying veins. Unlike the median and medial umbilical folds, the contents of the lateral umbilical fold remain functional after birth. It originates just medial to the deep inguinal ring to the arcuate line on the posterior surface of the anterior abdominal wall. Clinical significance The lateral umbilical fold is an important reference site with regards to hernia classification. A direct hernia occurs medial to the lateral umbilical fold, whereas an indirect hernia originates lateral to the fold. This latter case is due to the placement of the opening of the deep inguinal ring in the space lateral to the lateral umbilical fold, which allows the passage of the ductus deferens, testicular artery, and other components of the spermatic cord in men, or the round ligament of the uterus in women.
