Transport Safety Investigation Bureau
The Transport Safety Investigation Bureau (TSIB) is statutory board under the Ministry of Transport of the Government of Singapore and is an independent investigation authority, responsible for the investigation of air, marine and land transport accidents and incidents in Singapore. The head office is in Passenger Terminal 2, Changi Airport, Changi, Singapore. It was formed on 1 August 2016 as a restructuring of the Air Accident Investigation Bureau (AAIB) of Singapore. History The AAIB was set up in 2002 after the SilkAir Flight 185 and Singapore Airlines Flight 006 crashes. The bureau set up a facility in 2007 to analyze data from flight data recorders (informally known as "black boxes") installed on commercial aircraft. A flight recorder is an electronic recording device placed in an aircraft for the purpose of facilitating the investigation of aviation accidents and incidents. The device may often be referred to as a "black box", an outdated name which has become a misnomer—they are now required to be painted bright orange, to aid in their recovery after accidents. There are two types of flight recording devices: the flight data recorder (FDR) preserves the recent history of the flight through the recording of dozens of parameters collected several times per second; the cockpit voice recorder (CVR) preserves the recent history of the sounds in the cockpit, including the conversation of the pilots. The Convention on International Civil Aviation, also known as the Chicago Convention, established the International Civil Aviation Organization (ICAO), a specialized agency of the United Nations charged with coordinating international air travel. The Convention establishes rules of airspace, aircraft registration and safety, security, and sustainability, and details the rights of the signatories in relation to air travel. The Convention also contains provisions pertaining to taxation. The document was signed on December 7, 1944, in Chicago by 52 signatory states. It received the requisite 26th ratification on March 5, 1947, and went into effect on April 4, 1947, the same date that ICAO came into being. Singapore Airlines Flight 006 (SQ006/SIA006) was a scheduled Singapore Airlines passenger flight from Singapore Changi Airport to Los Angeles International Airport via Chiang Kai-shek International Airport (now Taiwan Taoyuan International Airport) in Taipei, Taiwan. On 31 October 2000, at 23:18 Taipei local time (15:18 UTC), the Boeing 747-412 operating the flight attempted to take off from the wrong runway at Taiwan Taoyuan International Airport during a typhoon. The aircraft crashed into construction equipment on the runway, killing 81 of the 179 people aboard. Ninety-eight initially survived the impact, but two passengers died later from injuries in hospital. As of 2022, the accident is the third-deadliest on Taiwanese soil. SilkAir Flight 185 was a scheduled international passenger flight operated by a Boeing 737-300 from Soekarno–Hatta International Airport in Jakarta, Indonesia to Changi Airport in Singapore that crashed into the Musi River near Palembang, Sumatra, on 19 December 1997, killing all 97 passengers and seven crew on board. The cause of the crash was independently investigated by two agencies in two countries: the U.S. National Transportation Safety Board (NTSB) and the Indonesian National Transportation Safety Committee (NTSC). The NTSB, which had jurisdiction based on Boeing's manufacture of the aircraft in the U.S., investigated the crash under lead investigator Greg Feith.
