Grant Liddle
Grant Winder Liddle (June 27, 1921 – June 29, 1989) was an American endocrinologist whose research focused largely on the hypothalamic–pituitary–adrenal axis. He was a professor at Vanderbilt University and chaired its Department of Medicine from 1968 to 1983. Biography Liddle was born on June 27, 1921, in American Fork, Utah. He attended the University of Utah, graduating in 1943, before moving to California to attend the UCSF School of Medicine. After graduating in 1948, he became a research fellow at the National Institutes of Health Metabolic Research Unit. From 1953 to 1956, he worked as a surgeon for the Section on Clinical Endocrinology at the National Heart, Lung, and Blood Institute. He left to join the faculty of the Vanderbilt University School of Medicine as chief of the school's Endocrine Service, and was appointed chairman of Vanderbilt's Department of Medicine in 1968. Dexamethasone Suppression Test
The dexamethasone suppression test (DST) is used to assess adrenal gland function by measuring how cortisol levels change in response to oral doses or an injection of dexamethasone. It is typically used to diagnose Cushing's syndrome. The DST was historically used for diagnosing depression, but by 1988 it was considered to be "at best, severely limited in its clinical ability" for this purpose. Physiology Dexamethasone is an exogenous steroid that provides negative feedback to the pituitary gland to suppress the secretion of adrenocorticotropic hormone (ACTH). Specifically, dexamethasone binds to glucocorticoid receptors in the anterior pituitary gland, which lie outside the blood–brain barrier, resulting in regulatory modulation. A paraneoplastic syndrome is a syndrome (a set of signs and symptoms) that is the consequence of a tumor in the body (usually a cancerous one), specifically due to the production of chemical signaling molecules (such as hormones or cytokines) by tumor cells or by an immune response against the tumor. Unlike a mass effect, it is not due to the local presence of cancer cells. Paraneoplastic syndromes are typical among middle-aged to older patients, and they most commonly present with cancers of the lung, breast, ovaries or lymphatic system (a lymphoma). Adrenocorticotropic hormone (ACTH; also adrenocorticotropin, corticotropin) is a polypeptide tropic hormone produced by and secreted by the anterior pituitary gland. It is also used as a medication and diagnostic agent.
