Gastric Mucosal Barrier
The gastric mucosal barrier is the property of the stomach that allows it to safely contain the gastric acid required for digestion. If the barrier is broken, as by acetylsalicylic acid (ASA, aspirin) in acid solution, acid diffuses back into the mucosa where it can cause damage to the stomach itself. The barrier consists of three protective components. These provide the additional resistance for the mucosal surface of the stomach. The three components include: * A compact epithelial cell lining. Cells in the epithelium of the stomach are bound by tight junctions that repel harsh fluids that may injure the stomach lining. * A special mucus covering, derived from mucus secreted by surface epithelial cells and Foveolar cells. This insoluble mucus forms a protective gel-like coating over the entire surface of the gastric mucosa. Gastric Acid
Gastric acid, gastric juice, or stomach acid is a digestive fluid formed within the stomach lining. With a pH between 1 and 3, gastric acid plays a key role in digestion of proteins by activating digestive enzymes, which together break down the long chains of amino acids of proteins. Gastric acid is regulated in feedback systems to increase production when needed, such as after a meal. Other cells in the stomach produce bicarbonate, a base, to buffer the fluid, ensuring a regulated pH. These cells also produce mucus – a viscous barrier to prevent gastric acid from damaging the stomach. The pancreas further produces large amounts of bicarbonate and secretes bicarbonate through the pancreatic duct to the duodenum to neutralize gastric acid passing into the digestive tract. Acetylsalicylic Acid
Aspirin, also known as acetylsalicylic acid (ASA), is a nonsteroidal anti-inflammatory drug (NSAID) used to reduce pain, fever, and/or inflammation, and as an antithrombotic. Specific inflammatory conditions which aspirin is used to treat include Kawasaki disease, pericarditis, and rheumatic fever. Aspirin is also used long-term to help prevent further heart attacks, ischaemic strokes, and blood clots in people at high risk. For pain or fever, effects typically begin within 30 minutes. Aspirin works similarly to other NSAIDs but also suppresses the normal functioning of platelets. One common adverse effect is an upset stomach. More significant side effects include stomach ulcers, stomach bleeding, and worsening asthma. Foveolar Cells
Foveolar cells or surface mucous cells are mucus-producing cells which cover the inside of the stomach, protecting it from the corrosive nature of gastric acid. These cells line the gastric mucosa (mucous neck cells are found in the necks of the gastric pits). The mucus-secreting cells of the stomach can be distinguished histologically from the intestinal goblet cells, another type of mucus-secreting cell. Structure The gastric mucosa that lines the inner wall of the stomach has a set of microscopic features called gastric glands which, depending on the location within the stomach, secrete different substances into the lumen of the organ. Nonsteroidal Anti-inflammatory Drug
Non-steroidal anti-inflammatory drugs (NSAID) are members of a therapeutic drug class which reduces pain, decreases inflammation, decreases fever, and prevents blood clots.
