Consultant (medicine)
In the United Kingdom, Ireland, and parts of the Commonwealth, consultant is the title of a senior hospital-based physician or surgeon who has completed all of their specialist training and been placed on the specialist register in their chosen speciality. Their role is entirely distinct from that of general practitioners, or GPs. The primary objective of a consultant is to use expert knowledge and skill to diagnose and treat patients while retaining ultimate clinical responsibility for their care. A physician must be on the Specialist Register before they may be employed as a substantive consultant in the National Health Service (NHS). Staff Grade
In the United Kingdom, a staff grade doctor or dentist (now called SAS grades) is one who is appointed to a permanent position. Since 2008 this grade was closed and new appointees were called as specialty doctors/dentists. They will have been appointed into this position based on experience and their CV and not all doctors will have done post-graduate examinations to reach this position. In the past, staff grades could be promoted to Associate Specialists after several years of service and development by either interview and/or personal promotion. They may also apply to take up consultant posts through the GMC CESR route, or the GDC mediated entry route, which allows them to be registered on the specialist register after assessment of their experience. Specialist Registrar
A specialist registrar (SpR) is a doctor in the United Kingdom or the Republic of Ireland who is receiving advanced training in a specialist field of medicine in order to become a consultant in that specialty. After graduation from medical school, a specialist registrar will undertake several years of work and training as an intern or pre-registration house officer, and as a senior house officer. It may be required to take diploma examinations in order to enter registrar training. These are administered by the medical royal college or in some cases a separate postgraduate training body responsible for that specialty and are usually termed "memberships". In the United Kingdom, junior doctors are qualified medical practitioners working whilst engaged in postgraduate training. Medical Fellowship
A fellowship is the period of medical training, in the United States and Canada, that a physician, dentist, or veterinarian may undertake after completing a specialty training program (residency). During this time (usually more than one year), the physician is known as a fellow. Fellows are capable of acting as an attending physician or a consultant physician in the specialist field in which they were trained, such as internal medicine or pediatrics. After completing a fellowship in the relevant sub-specialty, the physician is permitted to practice without direct supervision by other physicians in that sub-specialty, such as cardiology or oncology. Medical Residency
Residency or postgraduate training is specifically a stage of graduate medical education. It refers to a qualified physician (one who holds the degree of MD, DO, MBBS, MBChB), veterinarian ( DVM or VMD) , dentist ( DDS or DMD) or podiatrist ( DPM) who practices medicine, veterinary medicine , dentistry, or podiatry, respectively, usually in a hospital or clinic, under the direct or indirect supervision of a senior medical clinician registered in that specialty such as an attending physician or consultant. In many jurisdictions, successful completion of such training is a requirement in order to obtain an unrestricted license to practice medicine, and in particular a license to practice a chosen specialty. Modernising Medical Careers
Modernising Medical Careers (MMC) is a programme for postgraduate medical training introduced in the United Kingdom in 2005. The programme replaced the traditional grades of medical career before the level of Consultant. The different stages of the programme contribute towards a "Certificate of Completion of Training" (CCT). It has been dogged by criticism within and outside the medical profession, and an independent review of MMC led by Professor Sir John Tooke criticised many aspects of it. Foundation Doctor
A foundation doctor is a grade of medical practitioner in the United Kingdom undertaking the Foundation Programme, a two-year, general postgraduate medical training programme which forms the bridge between medical school and specialist/general practice training. Doctors in the first year of the programme are known as Foundation Year 1 (FY1) doctors, and those in the second year are known as Foundation Year 2 (FY2) doctors.
