Aviastar Flight 7503
Aviastar Flight 7503 was a regional flight from Masamba to Makassar, Indonesia. On 2 October 2015, the de Havilland Canada DHC-6 Twin Otter aircraft serving the route went missing with 10 on board near Palopo minutes after takeoff. There was no distress call from the plane. After an extensive search operation, three days later the plane was found crashed and it was confirmed that all 10 on board were dead. It was Aviastar's deadliest crash. National Transportation Safety Committee released the final report in January 2017 and concluded that the crash was caused by pilot error. They found out that both pilots agreed to deviate the plane from its designated route and decided to "take a shortcut", and thus cutting the travel time of the airplane. However, by doing so the plane have to pass the mountains on the middle of the route. This wouldn't have happened if they stayed at their designated track which was near the shoreline. Controlled Flight Into Terrain
In aviation, a controlled flight into terrain (CFIT; usually ) is an aviation accidents and incidents, accident in which an airworthy aircraft, under aircraft pilot, pilot control, is unintentionally flown into the ground, a mountain, a body of water or an obstacle. In a typical CFIT scenario, the aircrew, crew is unaware of the impending disaster until it is too late. The term was coined by engineers at Boeing in the late 1970s. Accidents where the aircraft is out of control at the time of impact, because of mechanical failure or pilot error, are not considered CFIT (they are known as ''uncontrolled flight into terrain'' or ''UFIT''), nor are incidents resulting from the deliberate action of the person at the controls, such as acts of terrorism or suicide by pilot. Trigana Air Service Flight 267
Trigana Air Flight 267 was a scheduled passenger flight from Sentani to Oksibil in the eastern Indonesian province of Papua. On 16 August 2015, the ATR 42 turboprop operating the service crashed on approach in the Bintang highlands region of Oksibil, killing all 49 passengers and 5 crew members. With 54 deaths, it is the deadliest accident involving the ATR 42, and the airline's deadliest accident since its establishment in 1991. The Indonesian National Transportation Safety Committee published its final report into the accident in December 2017. Deviation from visual approach guidance and visual flight rules without regard to weather and terrain, and the likely deactivation of the EGPWS system contributed directly to the crash, compounded by shortcomings in Trigana Air's safety culture. Mount Salak ( id, Gunung Salak, su, Gunung Salak) is an eroded volcano in West Java, Indonesia. It has several satellite cones on its southeast flank and the northern foot, along with two additional craters at the summit. Mount Salak has been evaluated for geothermal power development. According to a popular belief, the name "Salak" is derived from ''salak'', a tropical fruit with scaly skin; however, according to Sundanese tradition, the name was derived from the Sanskrit word ''Salaka'' which means "silver". Mount Salak Sukhoi Superjet 100 Crash
On 9 May 2012, a Sukhoi Superjet 100 airliner on a demonstration tour in Indonesia crashed into Mount Salak, in the province of West Java. All 37 passengers and 8 crew on board were killed. The plane had taken off minutes before from Jakarta's Halim Airport on a promotional flight for the recently launched jet, and was carrying Sukhoi personnel and representatives of various local airlines. The subsequent investigation concluded that the flight crew was unaware of the presence of high ground in the area and ignored warnings from the terrain warning system, incorrectly attributing them to a system malfunction while their view was obstructed because of thick cloud cover. It was also established that in the minutes leading to the accident, the crew, including the captain, were engaged in conversation with prospective customers present in the cockpit. Merpati Nusantara Airlines Flight 9760D was a domestic commercial passenger 50-minutes flight, flying from Sentani Airport in Papua's Province Jayapura to Oksibil Airport in Oksibil, Indonesia operated by a de Havilland Canada DHC-6 Twin Otter 300. On Sunday, August 2, 2009, while carrying fifteen people over Papua, the aircraft went missing en route. Its wreckage was found a few miles from Oksibil two days later. All 12 passengers and 3 crew members were killed in the accident. Indonesian Investigation Agency, NTSC released the final report and concluded that the cause of the crash was controlled flight into terrain (CFIT). A flight recorder is an electronic recording device placed in an aircraft for the purpose of facilitating the investigation of aviation accidents and incidents. Makassar Strait is a strait between the islands of Borneo and Sulawesi in Indonesia. To the north it joins the Celebes Sea, while to the south it meets the Java Sea.
