Form (document)
A form is a document which contains blank spaces (also named ''fields'' or ''placeholders'') in which one can write or select an option. Forms can be distributed to several signatories at once, or made available on demand. Before being filled out, each copy of a form is usually identical, except, possibly, for a serial number. A form allows an organisation to collect a uniform set of data from many parties in a consistent manner. Forms, when completed, vary in their purpose; for example, a form might be a statement, a request, or an order. A cheque may also be considered a form. In addition, there are several forms for taxes. An example is a tax return; filling one out is required in order for the amount of tax one owes to be determined. A form may also be a request for a tax refund. Enterprise Forms Automation
Enterprise forms automation is a company-wide computer system or set of systems for managing, distributing, completing, and processing paper-based forms, applications, surveys, contracts, and other documents. It plays a vital role in the concept of a paperless office. JetForm Corporation first used the term "enterprise forms automation" in the mid 1990s to describe their solution to automating paperwork. This process was later acquired by Adobe Systems, and is now part of the Adobe LiveCycle suite of products. Oracle Forms
Oracle Forms is a software product for creating screens that interact with an Oracle database. It has an IDE that includes an object navigator, property sheet, and code editor that uses PL/SQL. It was originally developed to run server-side in character-mode terminal sessions. It was ported to other platforms, including Windows, to function in a client–server environment. Later versions were ported to Java where it runs in a Java EE container and can integrate with Java, and web services that can be launched from a URL. A webform, web form or HTML form on a web page allows a user to enter data that is sent to a server for processing. Forms can resemble paper or database forms because web users fill out the forms using checkboxes, radio buttons, or text fields. XForms
XForms is an XML format used for collecting inputs from web forms. XForms was designed to be the next generation of HTML / XHTML forms, but is generic enough that it can also be used in a standalone manner or with presentation languages other than XHTML to describe a user interface and a set of common data manipulation tasks. Questionnaire
A questionnaire is a research instrument that consists of a set of questions (or other types of prompts) for the purpose of gathering information from respondents through survey or statistical study. A research questionnaire is typically a mix of close-ended questions and open-ended questions. Open-ended, long-term questions offer the respondent the ability to elaborate on their thoughts.
