Angry that he would take credit for Kenobi's capture, Reva stabs the Grand Inquisitor in the stomach. This also proves a long enough distraction for Kenobi to escape with Leia. Following their escape, Reva reports to

However, the original Grand Inquisitor is revealed to have survived Reva's betrayal, attributed to his thirst for revenge, and retakes his position as leader of the Inquisitorius when Reva is wounded by Vader as she attempts to assassinate the Dark Lord.
