Preputioplasty
Preputioplasty or prepuce plasty, also known as "limited dorsal slit with transverse closure", is a plastic surgical operation on the prepuce or foreskin of the penis, to widen a narrow non-retractile foreskin which cannot comfortably be drawn back off the head of the penis in erection because of a constriction ( stenosis) which has not expanded after adolescence. Preputioplasty is a treatment for phimosis in the alternative to circumcision and radical dorsal slit * which is conservative, non-traumatic and less invasive and can be performed on an outpatient basis under local anaesthetic in a doctor's office and * has the advantage of healing very quickly with little or no significant cosmetic alteration to the appearance of the penis. Methods of performing preputioplasty Preputioplasty may be performed by Z-plasty Z-plasty is a versatile plastic surgery technique that is used to improve the functional and cosmetic appearance of scars. Foreskin
In male human anatomy, the foreskin, also known as the prepuce, is the double-layered fold of skin, mucosal and muscular tissue at the distal end of the human penis that covers the glans and the urinary meatus. The foreskin is attached to the glans by an elastic band of tissue, known as the frenulum. The outer skin of the foreskin meets with the inner preputial mucosa at the area of the mucocutaneous junction. The foreskin is mobile, fairly stretchable and sustains the glans in a moist environment. Except for humans, a similar structure, known as penile sheath, appears in the male sexual organs of all primates and the vast majority of mammals. In humans, foreskin length varies widely and coverage of the glans in a flaccid and erect state can also vary. The foreskin is fused to the glans at birth and is generally not retractable in infancy and early childhood. Inability to retract the foreskin in childhood should not be considered a problem unless there are other symptoms. Retr ...

Phimosis
Phimosis (from Greek language, Greek φίμωσις ''phimōsis'' 'muzzling'.) is a condition in which the foreskin of the Human penis, penis cannot stretch to allow it to be pulled back past the Glans penis, glans. A balloon-like swelling under the foreskin may occur with urination. In teenagers and adults, it may result in pain during an erection, but is otherwise not painful. Those affected are at greater risk of inflammation of the glans, known as balanitis, and other complications. In young children, it is normal not to be able to pull back the foreskin at all. Over 90% of cases resolve by the age of seven, although full retraction is still prevented by preputial adhesions in over half at this age. Occasionally, phimosis may be caused by an underlying condition such as scarring due to balanitis or balanitis xerotica obliterans. This can typically be diagnosed by seeing scarring of the opening of the foreskin. Typically, the condition resolves without treatment by the age ...

Penis
A penis (plural ''penises'' or ''penes'' () is the primary sexual organ that male animals use to inseminate females (or hermaphrodites) during copulation. Such organs occur in many animals, both vertebrate and invertebrate, but males do not bear a penis in every animal species, and in those species in which the male does bear a so-called penis, the penises in the various species are not necessarily homologous. The term ''penis'' applies to many intromittent organs, but not to all. As an example, the intromittent organ of most cephalopoda is the hectocotylus, a specialized arm, and male spiders use their pedipalps. Even within the Vertebrata there are morphological variants with specific terminology, such as Hemipenis, hemipenes. In most species of animals in which there is an organ that might reasonably be described as a penis, it has no major function other than intromission, or at least conveying the sperm to the female, but in the Eutheria, placental mammals the peni ...

Stenosis
A stenosis (from Ancient Greek στενός, "narrow") is an abnormal narrowing in a blood vessel or other tubular organ or structure such as foramina and canals. It is also sometimes called a stricture (as in urethral stricture). ''Stricture'' as a term is usually used when narrowing is caused by contraction of smooth muscle (e.g. achalasia, prinzmetal angina); ''stenosis'' is usually used when narrowing is caused by lesion that reduces the space of lumen (e.g. atherosclerosis). The term coarctation is another synonym, but is commonly used only in the context of aortic coarctation. Restenosis is the recurrence of stenosis after a procedure. Types The resulting syndrome depends on the structure affected. Examples of vascular stenotic lesions include: * Intermittent claudication (peripheral artery stenosis) * Angina ( coronary artery stenosis) * Carotid artery stenosis which predispose to (strokes and transient ischaemic episodes) * Renal artery stenosis The types of sten ...

Adolescence
Adolescence () is a transitional stage of physical and psychological development that generally occurs during the period from puberty to adulthood (typically corresponding to the age of majority). Adolescence is usually associated with the teenage years, but its physical, psychological or cultural expressions may begin earlier and end later. Puberty now typically begins during preadolescence, particularly in females. Physical growth (particularly in males) and cognitive development can extend past the teens. Age provides only a rough marker of adolescence, and scholars have not agreed upon a precise definition. Some definitions start as early as 10 and end as late as 25 or 26. The World Health Organization definition officially designates an adolescent as someone between the ages of 10 and 19. Biological development Puberty in general Puberty is a period of several years in which rapid physical growth and psychological changes occur, culminating in sexual maturity. The aver ...

Circumcision
Circumcision is a surgical procedure, procedure that removes the foreskin from the human penis. In the most common form of the operation, the foreskin is extended with forceps, then a circumcision device may be placed, after which the foreskin is wikt:excise, excised. Topical or locally injected anesthesia is generally used to reduce pain and stress (physiology), physiologic stress. It is usually elective surgery, elective, performed as preventive healthcare, a Religious law, religious rite, or Culture, cultural practice. It is also an option for cases of phimosis, other Pathology, pathologies that do not resolve with other treatments, and chronic urinary tract infections (UTIs). The procedure is contraindicated in cases of certain genital structure abnormalities or poor general health. Circumcision is associated with reduced rates of sexually transmitted infections and urinary tract infections. This includes decreasing the incidence of Carcinogenesis, cancer-causing forms of ...

Dorsal Slit
A dorsal slit (often referred to in anthropology as superincision or supercision) is a single incision along the upper length of the foreskin from the tip to the corona, exposing the glans without removing any tissue. An ancient practice, it has been a traditional custom among a number of peoples, particularly Filipinos and Pacific Islanders, probably for thousands of years. In Western medicine, it was used as an alternative to circumcision to relieve conditions such as failure of the foreskin to retract (phimosis) or failure to cover the glans penis (paraphimosis), although a perception of poor appearance limited its popularity. While it is a less invasive surgery than circumcision, it is more invasive than preputioplasty since it leaves the incision open. It is still used when circumcision or other measures are considered impractical or undesirable. Traditional custom Since superincision and circumcision are both forms of genital cutting that expose the glans, it can often be dif ...

Z-plasty
Z-plasty is a versatile plastic surgery technique that is used to improve the functional and cosmetic appearance of scars. It can elongate a contracted scar or rotate the scar tension line. The middle line of the Z-shaped incision (the central element) is made along the line of the greatest tension or contraction, and triangular flaps are raised on opposite sides of the two ends and then transposed. The length and angle of each flap are usually the same to avoid mismatched flaps that may be difficult to close. Some possible complications of Z-plasty include flap necrosis, haematoma (blood clot) formation under the flaps, wound infection, trapdoor effect and sloughing (necrosis) of the flap caused by wound tension and inadequate blood supply. Classification Z-plasties can be functional (elongate and relax scars) or cosmetic (realign scars to make them less noticeable). They can be single or multiple. Variations include skew and planimetric Z-plasties. Technique The transposition ...

Male Genital Surgery
Male (Mars symbol, symbol: ♂) is the sex of an organism that produces the gamete (sex cell) known as sperm, which fuses with the larger female gamete, or ovum, in the process of fertilization. A male organism cannot sexual reproduction, reproduce sexually without access to at least one ovum from a female, but some organisms can reproduce both sexually and Asexual reproduction, asexually. Most male mammals, including male humans, have a Y chromosome, which codes for the production of larger amounts of testosterone to develop male reproductive organs. Not all species share a common sex-determination system. In most animals, including Homo sapiens, humans, sex is determined genetics, genetically; however, species such as ''Cymothoa exigua'' change sex depending on the number of females present in the vicinity. In humans, the word ''male'' can also be used to refer to gender in the social sense of gender role or gender identity. Overview The existence of separate sexes has evol ...
