Pimobendan
Pimobendan (INN, or pimobendane; tradenames Vetmedin, Acardi) is a veterinary medication. It is a calcium sensitizer and a selective inhibitor of phosphodiesterase 3 (PDE3) with positive inotropic and vasodilator effects. Pimobendan is used in the management of heart failure in dogs, most commonly caused by myxomatous mitral valve disease (also previously known as endocardiosis), or dilated cardiomyopathy. Research has shown that as a monotherapy, pimobendan increases survival time and improves quality of life in canine patients with congestive heart failure secondary to mitral valve disease when compared with benazepril, an ACE inhibitor. However, in clinical practice, it is often used in conjunction with an ACE inhibitor like enalapril or benazepril. Under the trade name Acardi, it is available for human use in Japan. PDE3 Inhibitor
A PDE3 inhibitor is a drug which inhibits the action of the phosphodiesterase enzyme PDE3. They are used for the therapy of acute heart failure and cardiogenic shock. Medical uses Cardiac Amrinone, milrinone and enoximone are used clinically for short-term treatment of cardiac failure in the presence of cardiogenic shock. PDE3 inhibitors are indicated as inotropics for the therapy of acute heart failure if catecholamines are ineffective. Well controlled studies have shown that these drugs generally increase mortality, when used for the therapy of acute heart failure, so they have to be applied under close observation. Levosimendan
Levosimendan (International Nonproprietary Name, INN) is a calcium sensitizer used in the management of acutely decompensated congestive heart failure. It is marketed under the trade name Simdax (Orion Corporation). Overall the drug has a two fold mechanism of action. It leads to greater ionotrophy by increasing the calcium sensitivity as it binds to Troponin and this results in a greater positive iontrophic force. Secondly, the drug is able to open ATP sensitive potassium channels in vascular smooth muscle cells, and the vascular dilatory effects of the drug lead to a decreased pre-load and afterload, putting less work on the heart.
