Commissioning Support Unit
Commissioning support units were established in April 2013 from the remains of the primary care trusts and strategic health authorities as part of the reorganisation of the National Health Service in England following the Health and Social Care Act 2012. Twenty five regional commissioning support units submitted outline business plans in 2012 to NHS England which hosted them. The commissioning support units were largely staffed by former employees of primary care trusts. They were intended to provide support to clinical commissioning groups by providing business intelligence, health and clinical procurement services and other back-office administrative functions, including contract management. The plan was to introduce competition into this market by making them independent businesses from 2016. By May 2014 there had been a number of amalgamations as some clinical commissioning groups brought their commissioning support services in-house. Greater East Midlands Commissioning Sup ... Primary Care Trust
Primary care trusts (PCTs) were part of the National Health Service in England from 2001 to 2013. PCTs were largely administrative bodies, responsible for commissioning primary, community and secondary health services from providers. Until 31 May 2011, they also provided community health services directly. Collectively PCTs were responsible for spending around 80 per cent of the total NHS budget. Primary care trusts were abolished on 31 March 2013 as part of the Health and Social Care Act 2012, with their work taken over by clinical commissioning groups. Establishment In 1997 the incoming Labour Government abolished GP Fundholding. In April 1999 they established 481 primary care groups in England "thereby universalising fundholding while repudiating the concept." Primary and community health services were brought together in a single Primary Care Group controlling a unified budget for delivering health care to and improving the health of communities of about 100,000 people. Strategic Health Authority
Strategic health authorities (SHA) were part of the structure of the National Health Service in England between 2002 and 2013. Each SHA was responsible for managing performance, enacting directives and implementing health policy as required by the Department of Health at a regional level. History In 2002, the existing regional health authorities were renamed and merged to form 28 new strategic health authorities, under the National Health Service Reform and Health Care Professions Act 2002. This was the first time that the regional tier in the NHS reported directly to the centre rather than having a board and non-executive members. The SHA chief executives were appointed after a wide-ranging search and selection process and did not represent continuity with the previous regions, not least because CEOs were allocated to areas they were not generally familiar with. A number of the new CEOs had previously been health authority CEOs or CEOs of large trusts, rather than being on the ... It provided for the most extensive reorganisation of the structure of the National Health Service in England to date.''BMJ'', 2011; 342:d408Dr Lansley's Monster It removed responsibility for the health of citizens from the Secretary of State for Health, which the post had carried since the inception of the NHS in 1948. It abolished primary care trusts (PCTs) and strategic health authorities (SHAs) and transferred between £60 billion and £80 billion of "commissioning", or healthcare funds, from the abolished PCTs to several hundred clinical commissioning groups, partly run by the general practitioners (GPs) in England. A new executive agency of the Department of Health, Public Health England, was established under the act on 1 April 2013. The proposals are primarily the result of policies of the then Secretary of State for Health, Andrew Lansley. Writing in the ''BMJ'', Clive Peedell (co-c ... Clinical Commissioning Group
Clinical commissioning groups (CCGs) were NHS organisations set up by the Health and Social Care Act 2012 to organise the delivery of NHS services in each of their local areas in England. On 1 July 2022 they were abolished and replaced by Integrated care systems as a result of the Health and Care Act 2022. Establishment The announcement that GPs would take over this commissioning role was made in the 2010 white paper "Equity and Excellence: Liberating the NHS". This was part of the government's stated desire to create a clinically-driven commissioning system that was more sensitive to the needs of patients. The 2010 white paper became law under the Health and Social Care Act 2012 in March 2012. At the end of March 2013 there were 211 CCGs, but a series of mergers had reduced the number to 135 by April 2020. To a certain extent they replaced primary care trusts (PCTs), though some of the staff and responsibilities moved to local authority public health teams when PCTs ceased to ... Continuing Healthcare
The English national framework for NHS continuing healthcare came into force on 1 October 2007 as a development in the light of the case of Coughlan which established that where a person's need is primarily for health care then the health service must fund the whole cost of nursing home placement. People who qualify are entitled to care paid for by the NHS, for which they do not have to pay, rather than social care, which is means-tested. Most of those who qualify need nursing home care. It is in the interests of local social services departments to establish entitlement to continuing healthcare as this relieves them of any financial responsibility. This system has existed in one form or another since the creation of the NHS. Payments until 2013 were administered by primary care trusts, and this was transferred to clinical commissioning groups. Many CCGs found towards the end of 2014 that they were in financial difficulties facing the combination of an ageing population with compl ... Community Interest Company
A community interest company (CIC, colloquially pronounced "kick") is a type of company introduced by the United Kingdom government in 2005 under the Companies (Audit, Investigations and Community Enterprise) Act 2004, designed for social enterprises that want to use their profits and assets for the public good. CICs are intended to be easy to establish, with all the flexibility and certainty of the company form, but with some special features to ensure they are working for the benefit of the community. They are overseen by the Regulator of Community Interest Companies. CICs have proved popular and some 10,000 were registered in the status's first ten years. Objectives A community interest company is a business with primarily social objectives whose surpluses are principally reinvested for that purpose in the business or the community, rather than being driven by the need to maximize profit for shareholders and owners.
