Stephen M. Pachuta
Stephen Michael Pachuta (born 1960) is a retired United States Navy rear admiral. He served as the 37th Chief of the United States Navy Dental Corps. Pachuta retired from the Navy in December 2017, after 32 years of military service. He currently serves as Dean of his alma mater, the West Virginia University School of Dentistry. Early life and education Stephen Pachuta was raised in Beckley, West Virginia, and attended Woodrow Wilson High School before enrolling at West Virginia University. He earned his D.D.S. from the West Virginia University School of Dentistry in 1985. While enrolled, he earned the Student Award for Clinical Competency and Professionalism. After he received his commission in the United States Navy, he completed his residency training at the Naval Postgraduate Dental School in 1994, and also earned a Master of Science in Health Sciences from George Washington University. National Provider Identifier
A National Provider Identifier (NPI) is a unique 10-digit identification number issued to health care providers in the United States by the Centers for Medicare and Medicaid Services (CMS). The NPI has replaced the Unique Physician Identification Number (UPIN) as the required identifier for Medicare services, and is used by other payers, including commercial healthcare insurers. The transition to the NPI was mandated as part of the Administrative Simplifications portion of the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Mandated uses HIPAA–covered entities such as providers completing electronic transactions, healthcare clearinghouses, and large health plans were required by regulation to use only the NPI to identify covered healthcare providers by May 23, 2007. The surface warfare insignia is a military badge of the United States Navy which is issued to U.S. Navy personnel who are trained and qualified to perform duties aboard United States surface warships. There are presently four classes of the surface warfare pin, being that of line, staff, special operations, and enlisted. The International College of Dentists was conceived in 1920 at a farewell party in Tokyo for Dr. Louis Ottofy when he was returning home to the United States after practicing dentistry in the Philippines and Japan for 23 years. A colleague, Dr. Tsurukichi Okumura, a Japanese dentist, also urged Dr. Ottofy to form an international organization. Six years later, at the Seventh International Dental Congress in Philadelphia, U.S. the group of dentists met again to finalize the concept of the ICD then on New Year's Eve of 1927 the College was announced with Drs. Ottofy and Okumura as the Co-Founders. The first President of the College was Andres O. Weber, from Havana, Cuba. Originally, 250 dentists from 162 countries accepted the Fellowship oath. Operation Southern Watch was an air-centric military operation conducted by the United States Department of Defense from Summer 1992 to Spring 2003. United States Central Command's Joint Task Force Southwest Asia (JTF-SWA) had the mission of monitoring and controlling the airspace south of the 32nd Parallel (extended to the 33rd Parallel in 1996) in southern and south-central Iraq during the period following the end of the 1991 Persian Gulf War until the 2003 invasion of Iraq.
