PSA Flight 182
Pacific Southwest Airlines (PSA) Flight 182 was a scheduled flight of Pacific Southwest Airlines from Sacramento to Los Angeles and San Diego. On September 25, 1978, the Boeing 727-214 serving the flight, registration N533PS, collided with a private Cessna 172 light aircraft, registration N7711G, over San Diego, California. It was Pacific Southwest Airlines' first fatal accident, and it remains the deadliest air disaster in California history. At the time, it was the deadliest air crash in American history, and would remain so until American Airlines Flight 191 in May of 1979. Both aircraft crashed into North Park, a San Diego neighborhood. PSA 182 struck just north of the intersection of Dwight and Nile streets, killing all 135 people aboard the aircraft and seven people on the ground in houses, including two children. The Cessna struck Polk Avenue between 32nd and Iowa streets, killing the two on board. Nine others on the ground were injured and twenty-two residences were d ...

Mid-air Collision
In aviation, a mid-air collision is an aviation accident, accident in which two or more aircraft come into unplanned contact during flight. Owing to the relatively high velocities involved and the likelihood of subsequent impact with the ground or sea, very severe damage or the total destruction of at least one of the aircraft usually results. The potential for a mid-air collision is increased by Aviation communication, miscommunication, mistrust, error in Air navigation, navigation, deviations from flight plans, lack of situational awareness, and the lack of Traffic collision avoidance system, collision-avoidance systems. Although a rare occurrence in general due to the vastness of open space available, collisions often happen near or at airports, where large volumes of aircraft are spaced more closely than in general flight. First recorded mid-air collision The first recorded collision between aircraft occurred at the "Milano Circuito Aereo Internazionale" meeting held betwee ...

In this role, the NTSB investigates and reports on aviation accidents and incidents, certain types of highway crashes, ship and marine accidents, pipeline incidents, bridge failures, and railroad accidents. The NTSB is also in charge of investigating cases of hazardous materials releases that occur during transportation. The agency is based in Washington, D.C. It has four regional offices, located in Anchorage, Alaska; Denver, Colorado; Ashburn, Virginia; and Seattle, Washington. The agency also operates a national training center at its Ashburn facility. History The origin of the NTSB was in the Air Commerce Act of 1926, which assigned the United States Department of Commerce responsibility for investigating domestic aviation accidents. Before the NTSB, the Federal Aviation Administration's (FAA; at the t ...

Mayday (Canadian TV Series)
''Mayday'', entitled ''Air Crash Investigation'' in Canada (alternatively known as ''Air Crash Investigations'' on Seven Network), New Zealand, South Africa, the United Kingdom (alternatively known as ''Air Crash: Disaster Revealed'' on 5Select), India, other Asian countries, and some European countries, and ''Air Emergency'', ''Air Disasters'', and ''Mayday: Air Disaster'' in the United States, is a Canadian documentary television program examining air crashes, near-crashes, hijackings, bombings, and other disasters. ''Mayday'' uses re-enactments and computer-generated imagery to reconstruct the sequence of events leading up to each disaster. In addition, survivors, aviation experts, retired pilots, and crash investigators are interviewed, to explain how the emergencies came about, how they were investigated, and how they might have been prevented. List Of Mayday Episodes
''Mayday'', known as ''Air Crash Investigation(s)'' outside of the United States and Canada and also known as ''Mayday: Air Disaster'' (The Weather Channel) or ''Air Disasters '' (Smithsonian Channel) in the United States, is a Canadian documentary television series produced by Cineflix that recounts air crashes, near-crashes, fires, hijackings, bombings, and other mainly flight-related disasters and crises. It reveals the events that led to each crisis or disaster, their causes as determined by the official investigating body or bodies, and the measures they recommended to prevent a similar incident from happening again. The programs use re-enactments, interviews, eyewitness testimony, computer-generated imagery, cockpit voice recordings, and official reports to reconstruct the sequences of events. This does not include "Crash of the Century", a 2005 90-minute special investigating the Tenerife airport disaster, as the program is not branded as ''Mayday'' by Cineflix, even th ...

Cockpit Voice Recording
A flight recorder is an electronic recording device placed in an aircraft for the purpose of facilitating the investigation of aviation accidents and incidents. The device may often be referred to as a "black box", an outdated name which has become a misnomer—they are now required to be painted bright orange, to aid in their recovery after accidents. There are two types of flight recording devices: the flight data recorder (FDR) preserves the recent history of the flight through the recording of dozens of parameters collected several times per second; the cockpit voice recorder (CVR) preserves the recent history of the sounds in the cockpit, including the conversation of the pilots. The two devices may be combined into a single unit. Together, the FDR and CVR objectively document the aircraft's flight history, which may assist in any later investigation. The two flight recorders are required by international regulation, overseen by the International Civil Aviation Organiza ...

Visual Flight Rules
In aviation, visual flight rules (VFR) are a set of regulations under which a pilot operates an aircraft in weather conditions generally clear enough to allow the pilot to see where the aircraft is going. Specifically, the weather must be better than basic VFR weather minima, i.e. in visual meteorological conditions (VMC), as specified in the rules of the relevant aviation authority. The pilot must be able to operate the aircraft with visual reference to the ground, and by visually avoiding obstructions and other aircraft. If the weather is less than VMC, pilots are required to use instrument flight rules, and operation of the aircraft will be primarily through referencing the instruments rather than visual reference. In a control zone, a VFR flight may obtain a clearance from air traffic control to operate as Special VFR. Requirements VFR require a pilot to be able to see outside the cockpit, to control the aircraft's altitude, navigate, and avoid obstacles and other aircraft ...

Instrument Landing System
In aviation, the instrument landing system (ILS) is a precision radio navigation system that provides short-range guidance to aircraft to allow them to approach a runway at night or in bad weather. In its original form, it allows an aircraft to approach until it is over the ground, within a of the runway. At that point the runway should be visible to the pilot; if it is not, they perform a missed approach. Bringing the aircraft this close to the runway dramatically increases the range of weather conditions in which a safe landing can be made. Other versions of the system, or "categories", have further reduced the minimum altitudes, runway visual ranges (RVRs), and transmitter and monitoring configurations designed depending on the normal expected weather patterns and airport safety requirements. ILS uses two directional radio signals, the ''localizer'' (108 to 112 MHz frequency), which provides horizontal guidance, and the ''glideslope'' (329.15 to 335 MHz frequency ...

Flight Instructor
A flight instructor is a person who teaches others to operate aircraft. Specific privileges granted to holders of a flight instructor qualification vary from country to country, but very generally, a flight instructor serves to enhance or evaluate the knowledge and skill level of an aviator in pursuit of a higher pilot's license, certificate or rating. United States Privileges A person who holds a flight instructor certificate (called a "certificated flight instructor" or CFI) is authorized to give training and endorsements required for and relating to: *a student, private, commercial or other pilot certificate; *the three hours of training with reference only to instruments in preparation for a private pilot certificate, note that this does not need to be a CFII. *an instrument rating, only if the CFI has an instrument instructor rating (CFII); This cannot be given by a "safety pilot". A safety pilot can only be used to help maintain instrument proficiency with an instrument- ...
