Jurong Health Connect
Jurong Health Connect is a community health project started in Jurong, Singapore. The project was initiated by then Acting Minister of Health Khaw Boon Wan, and was officially launched on 3 July 2004. Community Health
Community health refers to simple health services that are delivered by laymen outside hospitals and clinics. Community health is also the subset of public health that is taught to and practiced by clinicians. Community health volunteers and community health workers work with Primary Care Providers to facilitate entry into, exit from and utilization of the formal health system by community members. Community health volunteers are members of the local community who have considerable knowledge of the health services available to the community and are used to identify and link beneficiaries or those in need and the registered providers. Jurong
Jurong () is a major geographical region located at the south-westernmost point of the West Region of Singapore. Although mostly vaguely defined, the region's extent roughly covers the planning areas of Jurong East, Jurong West, Boon Lay, and Pioneer, along with Jurong Island in the Western Islands cluster and the southernmost portions of the Western Water Catchment. Khaw Boon Wan
Khaw Boon Wan (; born 8 December 1952) is a Singaporean former politician who served as Minister for Transport between 2015 and 2020, Minister for National Development between 2011 and 2015, and Minister for Health between 2003 and 2011. A member of the governing People's Action Party (PAP), he was the Member of Parliament (MP) representing the Moulmein division of Tanjong Pagar GRC between 2001 and 2006, and the Sembawang division of Sembawang GRC between 2006 and 2020. Tan Cheng Bock
Adrian Tan Cheng Bock ( zh, s=陈清木, p=Chén Qīngmù; born 26 April 1940) is a Singaporean politician and medical practitioner. He was the Member of Parliament (MP) for Ayer Rajah SMC between 1980 and 2001. He served as Secretary-General of Progress Singapore Party (PSP) between 2019 to 2021 and served as Progress Singapore Party (PSP) Chairman since 2021. Early life Tan was born on 26 April 1940 in Singapore. He came from a Hokkien-speaking family. Tan attended Radin Mas Primary School and Raffles Institution (RI). At RI, he was once the classmate of the future Prime Minister and Emeritus Senior Minister Goh Chok Tong. He graduated from the University of Singapore (now National University of Singapore) in 1968 with a Bachelor of Medicine and Surgery.
