High 5 Project
The High 5s Project is an international patient safety collaboration launched by the World Health Organization (WHO) in 2006. The project addresses concerns about patient safety around the world. The High 5s name derives from the Project's original intent to significantly reduce the frequency of 5 challenging patient safety problems in 5 countries over 5 years. Organization The countries that initiated the High 5s Project were Australia, Canada, Germany, the Netherlands, New Zealand, the United Kingdom, and the United States. France, Saudi Arabia, and Singapore have subsequently joined the Project. All the countries set up Lead Technical Agencies for coordination of national project activities. Patient Safety
Patient safety is a discipline that emphasizes safety in health care through the prevention, reduction, reporting and analysis of error and other types of unnecessary harm that often lead to adverse patient events. The frequency and magnitude of avoidable adverse events, often known as patient safety incidents, experienced by patients was not well known until the 1990s, when multiple countries reported significant numbers of patients harmed and killed by medical errors. Recognizing that healthcare errors impact 1 in every 10 patients around the world, the World Health Organization (WHO) calls patient safety an endemic concern. WHO Collaborating Centre
World Health Organization collaborating centres are institutions that work with the World Health Organization (WHO) in disciplines such as occupational health, food safety, and communicable disease prevention. There are over 700 such centres across 80 countries. Collaborating centres may be research institutes, parts of universities, or academies. Standard Operating Procedure
A standard operating procedure (SOP) is a set of step-by-step instructions compiled by an organization to help workers carry out routine operations. SOPs aim to achieve efficiency, quality output, and uniformity of performance, while reducing miscommunication and failure to comply with industry regulations. Some military services (e.g., in the U.S. and the UK) use the term standing (rather than ''standard'') operating procedure, since a military SOP refers to a unit's unique procedures, which are not necessarily standard to another unit. The word "standard" could suggest that only one (standard) procedure is to be used across all units. German Agency For Quality In Medicine

German Coalition For Patient Safety
The German Coalition for Patient Safety (APS) – in German "Aktionsbündnis Patientensicherheit (APS)", was established in 2005 and located in Bonn is a German non-profit association of organisations and individuals interested and involved in promotion of patient safety. Activities APS' multidisciplinary working groups develop recommendations for patient safety activities in in- and outpatient healthcare institutions.
