Golden Hour (medicine)
The golden hour is the period of time immediately after a traumatic injury during which there is the highest likelihood that prompt medical and surgical treatment will prevent death. While initially defined as an hour, the exact time period depends on the nature of the injury and can be more than or less than this duration. It is well established that the person's chances of survival are greatest if they receive care within a short period of time after a severe injury; however, there is no evidence to suggest that survival rates drop off after 60 minutes. Some have come to use the term to refer to the core principle of rapid intervention in trauma cases, rather than the narrow meaning of a critical one-hour time period. General concept Cases of severe trauma, especially internal bleeding, require surgical intervention. Complications such as shock may occur if the person is not managed appropriately and expeditiously. It therefore becomes a priority to transport people with severe ...

R Adams Cowley Shock Trauma Center
R Adams Cowley Shock Trauma Center (also referred to simply as Shock Trauma) is a free-standing Physical trauma, trauma hospital in Baltimore, Maryland and is part of the University of Maryland Medical Center. It was the first facility in the world to treat Shock (circulatory), shock. Shock Trauma was founded by R Adams Cowley, considered the father and major innovator of Trauma (medicine), trauma medicine. Early years While serving in the United States Army in France immediately following World War II, Cowley observed that many severe traumatic injuries could be stabilized if the patient could be transported to a military hospital where a surgeon was present within one hour of the initial injury. Cowley coined the term "Golden hour (medicine), golden hour" to describe this crucial period of time. Cowley thus lobbied the legislature in Maryland to purchase helicopters for the transport of trauma patients to expedite their arrival to these higher-care facilities. The Maryland leg ...

Window Of Opportunity
A window of opportunity (also called a margin of opportunity or critical window) is a period of time during which some action can be taken that will achieve a desired outcome. Once this period is over, or the "window is closed", the specified outcome is no longer possible. Examples Examples of windows of opportunity include: *The critical period in neurological development, during which neuroplasticity is greatest and key functions, such as imprinting and language, are acquired which may be impossible to acquire at a later stage *The golden hour or golden time, used in emergency medicine to describe the period following traumatic injury in which life-saving treatment is likely to be successful * Market opportunities, in which one may be positioned to take advantage of a gap in a particular market, the timing of which may depend on the activities of customers, competitors, and other market context factors *Planting and harvesting seasons, in agriculture, which are generally ti ...

Air Ambulance
Air medical services is a comprehensive term covering the use of air transportation, aeroplane or helicopter, to move patients to and from healthcare facilities and accident scenes. Personnel provide comprehensive prehospital and emergency and critical care to all types of patients during aeromedical evacuation or rescue operations aboard helicopter and propeller aircraft or jet aircraft. The use of air transport to provide medical evacuation on the battlefield dates to World War I, but its role was expanded dramatically during the Korean and Vietnam wars. Later on, aircraft began to be used for the civilian emergency medical services. Helicopters can bring specialist care to the scene and transport patients to specialist hospitals, especially for major trauma cases. Fixed-wing aircraft are used for long-distance transport. In some remote areas, air medical services deliver non-emergency healthcare such as general practitioner appointments. An example of this is the Royal Flyin ...

World War I
World War I (28 July 1914 11 November 1918), often abbreviated as WWI, was one of the deadliest global conflicts in history. Belligerents included much of Europe, the Russian Empire, the United States, and the Ottoman Empire, with fighting occurring throughout Europe, the Middle East, Africa, the Pacific, and parts of Asia. An estimated 9 million soldiers were killed in combat, plus another 23 million wounded, while 5 million civilians died as a result of military action, hunger, and disease. Millions more died in genocides within the Ottoman Empire and in the 1918 influenza pandemic, which was exacerbated by the movement of combatants during the war. Prior to 1914, the European great powers were divided between the Triple Entente (comprising France, Russia, and Britain) and the Triple Alliance (containing Germany, Austria-Hungary, and Italy). Tensions in the Balkans came to a head on 28 June 1914, following the assassination of Archduke Franz Ferdin ...

Physical Trauma
An injury is any physiological damage to living tissue caused by immediate physical stress. An injury can occur intentionally or unintentionally and may be caused by blunt trauma, penetrating trauma, burning, toxic exposure, asphyxiation, or overexertion. Injuries can occur in any part of the body, and different symptoms are associated with different injuries. Treatment of a major injury is typically carried out by a health professional and varies greatly depending on the nature of the injury. Traffic collisions are the most common cause of accidental injury and injury-related death among humans. Injuries are distinct from chronic conditions, psychological trauma, infections, or medical procedures, though injury can be a contributing factor to any of these. Several major health organizations have established systems for the classification and description of human injuries. Occurrence Injuries may be intentional or unintentional. Intentional injuries may be acts o ...

R Adams Cowley
R Adams Cowley (July 25, 1917 – October 27, 1991) was an American surgeon considered a pioneer in emergency medicine and the treatment of shock trauma. Called the "Father of Trauma Medicine", he was the founder of the United States' first trauma center at the University of Maryland in 1958, after the US Army awarded him $100,000 to study shock in people—the first award of its kind in the United States. The trauma unit at first consisted of two beds, and was later expanded to four beds. Many people called the four-bed unit the "death lab." Cowley was the creator of the " Golden Hour" concept, the period of 60 minutes or less following injury when immediate definitive care is crucial to a trauma patient's survival. He was a leader in the use of helicopters for medical evacuations of civilians, beginning in 1969, and founded the Society of Thoracic Surgeons. He also founded the nation's first statewide EMS system, called MIEMSS by Executive Order of Maryland's Governor Mandel, 1972 ...

Emergency Medical Services
Emergency medical services (EMS), also known as ambulance services or paramedic services, are emergency services that provide urgent pre-hospital treatment and stabilisation for serious illness and injuries and transport to definitive care. They may also be known as a first aid squad, FAST squad, emergency squad, ambulance squad, ambulance corps, life squad or by other initialisms such as EMAS or EMARS. In most places, the EMS can be summoned by members of the public (as well as medical facilities, other emergency services, businesses and authorities) via an emergency telephone number which puts them in contact with a control facility, which will then dispatch a suitable resource for the situation. Ambulances are the primary vehicles for delivering EMS, though some also use squad cars, motorcycles, aircraft, or boats. EMS agencies may also operate a non-emergency patient transport service, and some have rescue squads to provide technical rescue services. As a first resort ...
