Five Wishes
Five Wishes is a United States advance directive created by the non-profit organization Aging with Dignity. It has been described as the "living will with a heart and soul". History Five Wishes was originally introduced in 1996 as a Florida-only document, combining a living will and health care power of attorney in addition to addressing matters of comfort care and spirituality. With help from the American Bar Association's Commission on Law and Aging and leading medical experts, a national version of Five Wishes was introduced in 1998. It was originally distributed with support from a grant by the Robert Wood Johnson Foundation. With assistance from the United Health Foundation, Five Wishes is now available in 28 languages and in Braille. More than 40 million documents have been distributed by a network of over 40,000 partner organizations worldwide. Aging with Dignity is a national non-profit organization based in Tallahassee, Florida. The stated mission of Aging with Dignity is to "honor the God-given human dignity of the most vulnerable among us". The primary focus of Aging with Dignity is to improve end-of-life care by encouraging people to make medical decisions in advance of a serious illness. History Aging with Dignity was founded in 1996 by Jim Towey, who was inspired by his twelve years of work as legal counsel for Mother Teresa of Calcutta. In the field of medicine, a healthcare proxy (commonly referred to as HCP) is a document (legal instrument) with which a patient (primary individual) appoints an agent to legally make healthcare decisions on behalf of the patient, when the patient is incapable of making and executing the healthcare decisions stipulated in the proxy. Once the healthcare proxy is effective, the agent continues making healthcare decisions as long as the primary individual is legally competent to decide. Moreover, in legal-administrative functions, the healthcare proxy is a legal instrument akin to a "springing" healthcare power of attorney. The proxy must declare the healthcare agent who will gain durable power attorney. Braille (Pronounced: ) is a tactile writing system used by people who are visually impaired, including people who are Blindness, blind, Deafblindness, deafblind or who have low vision.
