Superior Sagittal Sinus
The superior sagittal sinus (also known as the superior longitudinal sinus), within the human head, is an unpaired dural venous sinus lying along the attached margin of the falx cerebri. It allows blood to drain from the lateral aspects of the anterior cerebral hemispheres to the confluence of sinuses. Cerebrospinal fluid drains through arachnoid granulations into the superior sagittal sinus and is returned to the venous circulation. Structure It is triangular in section. It is narrower anteriorly, and gradually increases in size as it passes posterior-ward. It commences at the foramen cecum, through which it receives emissary veins from the nasal cavity. It passes posterior-ward along its entire course. The ''optic foramen'' is the opening to the optic canal. The canal is located in the sphenoid bone; it is bounded medially by the body of the sphenoid and laterally by the lesser wing of the sphenoid. The superior surface of the sphenoid bone is bounded behind by a ridge, which forms the anterior border of a narrow, transverse groove, the chiasmatic groove (optic groove), above and behind which lies the optic chiasma; the groove ends on either side in the optic foramen, which transmits the optic nerve and ophthalmic artery (with accompanying sympathetic nerve fibres) into the orbital cavity. Compared to the optic nerve, the ophthalmic artery is located inferolaterally within the canal. The left and right optic canals are 25mm apart posteriorly and 30mm apart anteriorly. The sphenoid bone is an unpaired bone of the neurocranium. It is situated in the middle of the skull towards the front, in front of the basilar part of occipital bone, basilar part of the occipital bone. The posterior ethmoidal nerve is a nerve of the head. It is a branch of the nasociliary nerve (itself a branch of the Ophthalmic nerve, ophthalmic nerve (CN V1)). It provides sensory innervation to the sphenoid sinus and ethmoid sinus, and part of the dura mater in the anterior cranial fossa. Structure Origin The posterior ethmoidal nerve is a branch of the nasociliary nerve. Course It passes through the posterior ethmoidal foramen alongside the posterior ethmoidal artery. Branches Within the anterior cranial fossa, it issues a branch to which innervates part of the dura mater. It gives branches to the sphenoid sinus and the ethmoid sinus. Variation The posterior ethmoidal nerve is absent in a significant proportion of people. This may be around 30%. The superior ophthalmic vein is a vein of the orbit that drains venous blood from structures of the upper orbit. It is formed by the union of the angular vein, and supraorbital vein. It passes backwards within the orbit alongside the ophthalmic artery, then exits the orbit through the superior orbital fissure to drain into the cavernous sinus. The superior ophthalmic vein can be a path for the spread of infection from the danger triangle of the face to the cavernous sinus and the pterygoid plexus. It may also be affected by an arteriovenous fistula of the cavernous sinus.
