Blunt Cardiac Injury
A blunt cardiac injury is an injury to the heart as the result of blunt trauma, typically to the anterior chest wall. It can result in a variety of specific injuries to the heart, the most common of which is a myocardial contusion, which is a term for a bruise (contusion) to the heart after an injury. Other injuries which can result include septal defects and valvular failures. The right ventricle is thought to be most commonly affected due to its anatomic location as the most anterior surface of the heart. Myocardial contusion is not a specific diagnosis and the extent of the injury can vary greatly. Usually, there are other chest injuries seen with a myocardial contusion such as rib fractures, pneumothorax, and heart valve injury. A bundle branch block is a defect in one the bundle branches in the electrical conduction system of the heart. Anatomy and physiology The heart's electrical activity begins in the sinoatrial node (the heart's natural pacemaker), which is situated on the upper right atrium. The impulse travels next through the left and right atria and summates at the atrioventricular node. Commotio cordis (Latin, "agitation of the heart") is an often lethal disruption of heart rhythm that occurs as a result of a blow to the area directly over the heart (the precordial region) at a critical time during the cycle of a heart beat, producing what is termed an R-on-T phenomenon that leads to the condition. It is a form of ventricular fibrillation (V-Fib), not mechanical damage to the heart muscle or surrounding organs, and not the result of heart disease. Pericardiocentesis (PCC), also called pericardial tap, is a medical procedure where fluid is aspirated from the pericardium (the sac enveloping the heart). Tamponade
Tamponade () is the closure or blockage (as of a wound or body cavity) by or as if by a tampon, especially to stop bleeding. Tamponade is a useful method of stopping a hemorrhage. This can be achieved by applying an absorbent dressing directly into a wound, thereby absorbing excess blood and creating a blockage, or by applying direct pressure with a hand or a tourniquet. There can, however, be disastrous consequences when tamponade occurs as a result of health problems, as in the case of cardiac tamponade. In this situation, fluid collects inside the pericardial sac. The pressure within the pericardium prevents the heart from expanding fully and filling the ventricles, with the result that a significantly reduced amount of blood circulates within the body. Ventricular Fibrillation
Ventricular fibrillation (V-fib or VF) is an abnormal heart rhythm in which the ventricles of the heart quiver. It is due to disorganized electrical activity. Ventricular fibrillation results in cardiac arrest with loss of consciousness and no pulse. This is followed by sudden cardiac death in the absence of treatment. Ventricular fibrillation is initially found in about 10% of people with cardiac arrest.
