Aircraft Accident Investigation Commission
The Aircraft Accidents Investigation Commission (AAIC, 航空事故調査委員会 ''Kōkūjiko chōsa iinkai'') was a government agency of Japan which investigated aviation accidents and incidents. It was subordinate to the Ministry of Transport, and after January 2001 the Ministry of Land, Infrastructure, Transport and Tourism (MLIT). It was founded in 1974, following the All Nippon Airways Flight 58 Shizukuishi aircraft accident on July 30, 1971, and the Toa Domestic Airlines Flight 63 accident. Japan Airlines Flight 350
was a McDonnell Douglas DC-8-61, registered ''JA8061'', on a domestic scheduled passenger flight from Fukuoka, Fukuoka Prefecture, to Tokyo in Japan. The airplane crashed 9 February 1982 on approach to Haneda Airport in Tokyo Bay, resulting in 24 fatalities. Flight 350 was the first crash for Japan Air Lines in the 1980s. The investigation traced the cause of the crash to the deliberate actions of the captain. Japan Air System Flight 451
Japan Air System Flight 451 was a Japan Air System flight from Nagoya Airport in the Nagoya area of Aichi Prefecture, Japan to New Chitose Airport in Sapporo, Hokkaido Prefecture, with a stopover at Hanamaki Airport in Hanamaki, Iwate Prefecture. On April 18, 1993, the Douglas DC-9-41 operating the flight crashed while landing at Hanamaki Airport. The aircraft suddenly lost a significant amount of airspeed as it crossed the boundary line of a passing cold front, and encountered resultant windshear while on final approach. The somewhat inexperienced first officer was not able to conduct a missed approach fast enough to avoid a hard landing. The plane then skidded off the runway. Japan Air Lines Flight 123 (JAL123) () was a scheduled domestic Japan Air Lines passenger flight from Haneda Airport in Tokyo to Itami International Airport in Osaka. On August 12, 1985, the Boeing 747SR operating this flight suffered a sudden decompression 12 minutes into the flight, and crashed in the area of Mount Takamagahara, Ueno, Gunma Prefecture, from Tokyo 32 minutes later. The crash site was on Osutaka Ridge, near Mount Osutaka. Garuda Indonesia Flight 865
Garuda Indonesia Flight 865 (GA/GIA 865) was a scheduled international flight from Fukuoka, Japan, to Jakarta, Indonesia via Bali, Indonesia. On 13 June 1996, Flight 865 crashed on its takeoff from runway 16 at Fukuoka Airport. Three of the 275 on board suffered fatal injuries in the accident. Accident Flight 865 was cleared for takeoff from Runway 16.
