Pilot Error
Pilot error generally refers to an Aviation accidents and incidents, accident in which an action or decision made by the Aircraft pilot#Airline, pilot was the cause or a contributing factor that led to the accident, but also includes the pilot's failure to make a correct decision or take proper action. Errors are intentional actions that fail to achieve their intended outcomes. Chicago Convention on International Civil Aviation, Chicago Convention defines accident as "An occurrence associated with the operation of an aircraft [...] in which [...] a person is fatally or seriously injured [...] ''except when the injuries are [...] inflicted by other persons."'' Mohammad Hussein Fadlallah
Grand Ayatollah Muhammad Husayn Fadlallah (also Sayyed Muhammad Hussein Fadl-Allāh; ar, محمد حسين فضل الله; 16 November 1935 – 4 July 2010) was a prominent twelver Shia cleric from a Lebanese family. Born in Najaf, Iraq, Fadlallah studied Islam in Najaf before moving to Lebanon in 1952. In the following decades, he gave many lectures, engaged in intense scholarship, wrote dozens of books, founded several Islamic religious schools, and established the ''Mabarrat Association''. Through the aforementioned association, he established a public library, a women's cultural center, and a medical clinic. Fadlallah was sometimes called the "spiritual mentor" of Hezbollah in the media, although this was disputed by other sources. He was also the target of several assassination attempts, including the 1985 Beirut car bombing. Pilot Fatigue
The International Civil Aviation Organization (ICAO) defines fatigue as "A physiological state of reduced mental or physical performance capability resulting from sleep loss or extended wakefulness, circadian phase, or workload." The phenomenon places great risk on the crew and passengers of an airplane because it significantly increases the chance of pilot error. Fatigue is particularly prevalent among pilots because of "unpredictable work hours, long duty periods, circadian disruption, and insufficient sleep". These factors can occur together to produce a combination of sleep deprivation, circadian rhythm effects, and 'time-on task' fatigue. Regulators attempt to mitigate fatigue by limiting the number of hours pilots are allowed to fly over varying periods of time. Beirut–Rafic Hariri International Airport
Beirut–Rafic Hariri International Airport (Arabic: مطار رفيق الحريري الدولي بيروت, (previously known as Beirut International Airport) () is the only operational commercial airport in Lebanon, which is located in the Southern Suburbs of Beirut, Lebanon, from the city center. It is the hub for Lebanon's national carrier, Middle East Airlines (MEA) and was the hub for the Lebanese cargo carrier TMA cargo and Wings of Lebanon before their respective collapses. The airport is named after former Lebanese Prime Minister Rafic Hariri in 2005, who was assassinated earlier that year. It is the main port of entry into the country along with the Port of Beirut. Ethiopian Airlines Flight 409
Ethiopian Airlines Flight 409 was an international commercial flight scheduled from Beirut to Addis Ababa that crashed into the Mediterranean Sea shortly after takeoff from Rafic Hariri International Airport on 25 January 2010, killing all 90 people on board. This was the first fatal crash for Ethiopian Airlines since the hijack of Ethiopian Airlines Flight 961 in 1996. Aircraft and crew The aircraft involved was a Boeing 737-8AS, registration ET-ANB, s/n 29935. It had its maiden flight on 18 January 2002, and was delivered new to Ryanair on 4 February 2002 as EI-CSW. Stored in , Ethiopian Airlines took delivery of the aircraft on 12 September 2009, leased from CIT Aerospace.
